What this website collects, who handles it, and how to have it deleted. Plain language, no small print.

What the form collects

When you fill in the form on the home page we receive what you type into it:

• your name, work email, company and role
• when your next questionnaire is due and roughly how many you get per year
• an optional message

We use it only to reply to you about your questionnaire and the pilot. We don't sell it and we don't add you to a newsletter.

Who processes it

• Tally (tally.so) hosts the form and stores the submissions until we delete them.
• Cloudflare serves this website and, like any host, sees ordinary request information such as your IP address and browser type.
• If you email us, your email provider and ours handle that message.

This website sets no cookies of its own and runs no advertising or analytics trackers. Tally's form may set its own cookies when it loads; see Tally's privacy policy.

Documents you send us

If you send policies or a questionnaire during the pilot, they are used only to fill your questionnaires. How we handle them is described in the “How we handle your documents” section of the home page. We'll sign your NDA first if you ask.

Ask for deletion

Email hello@citesheet.com from the address you used and say what to delete. We remove your form submission and any files you sent, and reply to confirm. You can also ask us what we hold about you.
